Person-Centred Planning
Person Outcome Measures® and Person-Centered Planning strategies represents the primary tools the agency uses to implement its philosophy. This is vital that every person supported has an avenue to discover his/ her own goals. The focus is to ensure effective planning, plan implementation, and coordination of services. This is accomplished by starting all plans with a strong vision for the person’s future.
Passport Services
Provide support to develop a plan and budget for your Passport funding, help link you to services in your community, and will follow up with you.
What you want, can be given to you!  Assist and help you uniquely design your supports.
Interdependent Community Supports
This is a holistic service that is flexible to ensure that the services provided reflect the direction given by the person supported. The level and area of support relate to the person’s plan that outlines the supports desired. The focus for supports provided also reflects the philosophy of the Association with specific emphasis on full participation in community life. The areas of support could be in one or all of the following: employment, medical, social, recreational, relationships, financial, education, planning, interpreting, in-home supports, respite, and community. These services are provided by Support Coordinators and Support Workers.
